All orders must be placed by phone. We are no longer accepting orders by facsimile (fax).

- First and Last Name
- Name of Organization (if applicable)
- Purchase Order Number (if your organization requires purchase orders)
- Shipping Address
- Billing Address (if different from shipping address)
- County of Residence
- Telephone Number
- Item Names and Quantities of Each
Payment Information: Payments must be made by check for education materials. At the moment, we cannot accept credit card payments for materials. An invoice will be sent along with your order that includes the appropriate amount of North Carolina sales tax.
